AY 2024-2025 Rotation Dates

The application has two parts: 

  1. Submission of this online form

    Please note that the form does not allow you to save your work. Please complete your work in a separate document before completing the online form.

  2. After you submit the form, you will receive a link via email to upload the following documents:

    • Curriculum Vitae (Name the file LastName_CV) 

    • Medical School Transcript - unofficial transcript is acceptable (LastName_Transcript) 

    • Personal Statement (LastName_PS)

The personal statement should include: 

  • Description of your goals for an advanced Family Medicine rotation, and how you believe these goals would be met by a rotation at our site 

  • Description of your interest in working with underserved populations 

  • Area(s) of interest in Family Medicine

 

Application deadline is March 22, 2024

  1. Who is eligible to apply for a sub-internship?

    • We welcome rising 4th year students at US medical schools to apply for a sub-internship

  2. How long should the personal statement be?

    • Maximum length should be 1 page, single spaced, font size 12

  3. How come I haven’t received a link to upload my documents?

    • The link has to be generated manually, so please give us 48-72 hours

  4. The link required a code to verify my email, but I have not yet received it - what should I do?

    • Please check your spam inbox;

    • If you are still experiencing issues, email the documents directly to us at SwedCherryHill_MedStudents@providence.org

  5. When will I hear back about the sub-i?

    • We will aim to notify applicants around early - mid April
